Transitions

The Transitions program is for teens and young adults ages 16 to 21 years that have autism and/or other disabilities. The purpose of this program is to provide skills training and support to help you navigate entrance into adulthood. If necessary, caregiver/parent training and support will be provided. Our goal is to work together with you, and potentially your parent(s), to create a plan based on what’s relevant to YOU.


Example areas of focus may include, but are not limited to:

  • vocational (i.e. work) skills

  • communication

  • navigating dating and sexuality

  • meal preparation

  • self-care

  • reducing harmful behavior

  • community involvement

  • leisure skills

  • interpersonal skills
